Because once you remove it, there is no going back.In this real patient example, you will see how Dr. Keith Gressell was able to save healthy tooth structure by utilizing "opportunistic access." What's that?Usually, when there is decay between teeth, the only way to treat it is through the chewing surface of the tooth. But not in this case...
In picture one, you can see a cavity on the backside of a tooth - next to a tooth with an old filling that needs to be replaced.Since we needed to replace the filling next to the new cavity, Dr. Gressell was able to use the opportunistic access approach.He accessed the new cavity by utilizing the replacement of the existing filling (see picture two). No healthy tooth structured was harmed in the making!Typically, we need to go through the chewing surface and extend the prep around the sides to allow for the matrix (the form we use to restore the tooth) and access to place the filling materials.Creating this access meant the prep was less than half the size it would have been for a traditional preparation! Ther creates a final restoration that is almost impossible to see (see picture three).In the final x-ray (picture four), you can see the chewing surface was untouched. The only way to see the filling is on the x-ray.Whenever the situation presents itself, we try our best to save every bit of tooth structure (and your money)!
